Who we are
Annandale Village Pharmacy, 107 Johnston Street, Annandale NSW 2038, is a community pharmacy and a member of the MediADVICE pharmacy network. We are bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and the NSW Health Records and Information Privacy Act 2002. Because we handle health information, the higher standards that apply to sensitive information apply to almost everything we hold about you.
What we collect
To dispense medicines and provide pharmacy services safely we may collect your name, contact details, date of birth, Medicare and concession card numbers, prescriptions and dispensing history, allergies and adverse reactions, vaccination records, and clinical information you give us during services such as UTI consultations, health checks, Webster packing and Home Medicines Reviews. If you order through our website, by email or through MedAdvisor, we also collect the details you submit there.
How we use it
We use your information to dispense your medicines, check for interactions and allergies, provide the service you have asked for, claim on your behalf under the PBS and Medicare, maintain your Safety Net record, and contact you about your medicines, such as when a script is ready or a repeat is due. We do not sell personal information and we do not use health information for marketing.
Who we share it with
Only where the service requires it or the law requires it: your prescriber or other treating health professionals; Services Australia for PBS and Medicare claims; the Australian Immunisation Register for vaccinations we administer; My Health Record and electronic prescription systems where you use them; NSW Health where reporting is mandated; and our dispensing and ordering software providers, who are bound by their own privacy obligations. If you ask us to transfer scripts to or from another pharmacy, we share what is needed to do that.
How we protect it
Records are held in secured dispensing systems with access limited to our pharmacists and trained staff, who are bound by professional confidentiality obligations. Paper records are stored securely and destroyed securely. Consultations that involve personal health matters happen in a private consultation room if you prefer.
Our website
Browsing this website does not require you to identify yourself. Forms on the site (such as sending us a script) collect only what you enter, and it is used solely to provide that service. Our booking links go to MedAdvisor, which operates under its own privacy policy.
Access and correction
You can ask to see the personal information we hold about you and ask us to correct it. Ask the pharmacist in store, call (02) 9660 1276 or email annandale@mediadvice.com.au. We will respond within a reasonable time and explain any grounds if we cannot provide access.
Complaints
If you believe we have mishandled your information, tell us first and we will investigate and respond. If you are not satisfied with our response, you can complain to the Office of the Australian Information Commissioner (oaic.gov.au, 1300 363 992) or, for NSW health records, the NSW Information and Privacy Commission.
