What it is
A Home Medicines Review (HMR) is a government-funded service for people managing several medicines at once. An accredited pharmacist visits you at home, goes through everything you are taking, including prescribed, over-the-counter and herbal medicines and the half-finished bottle in the cupboard, and writes a report for your GP with clear recommendations.
It catches things a five-minute counter conversation cannot: duplicated medicines from two different prescribers, doses that no longer suit, interactions, and medicines that stopped being necessary years ago.
How we arrange it
Your GP refers you, and we help make it happen. We do not conduct the review in-house. It is carried out by an external accredited pharmacist we refer to, which is how the service is designed to work. We will talk you through whether an HMR is likely to help, give you something to take to your GP, and coordinate the referral from our end.
Once the review is done, the report goes to your GP, and we work with whatever changes come out of it, including repacking your Webster packs to match.
Who it suits
People on five or more medicines, anyone recently out of hospital, anyone seeing multiple specialists, and anyone whose medicines have quietly accumulated over the years. We arrange reviews for patients across Annandale, Leichhardt, Stanmore and the wider Inner West.

Does a Home Medicines Review cost anything?
It is funded by Medicare when your GP refers you, so there is generally no cost to you.
Do you do the review yourselves?
No. We refer you to an external accredited pharmacist who conducts the visit. That is how the HMR programme is structured. We help arrange it and act on the outcome.
How do I get a referral?
Ask your GP, or come and talk to us first. We will tell you whether you are likely to be eligible and give you something to take to the appointment.
How long does the visit take?
Usually around an hour. Have all your medicines out on the table, including anything you buy without a script.
What happens afterwards?
The pharmacist sends a report to your GP with recommendations. Your GP decides what changes to make, and we update your dispensing and packs accordingly.
